NEGATIVE DECLARATION OF ENVIRONMENTAL IMPACT
APPLICANT: Henry Melendez, Dynamic Investments
LOCATION: 3630 East Temple Way
REQUEST: The project consists of a proposal to construct four single-family detached residential units on a 33,566-square foot lot located at the northeast corner of Holt Avenue and Temple Way. In order to facilitate the development of the site, following approvals are required:

(1) General Plan Amendment No. 03-02 to change the site's General Plan Designation from "Suburban Residential" (2.1 - 4.0 dwelling units per Acre) to "Low Medium Residential" (4.1 - 8.0 dwelling units per acre).

(2) East Hills Master Plan Amendment No. 13 to change the land use designation of the Master Plan from "Suburban Residential" to "Low Medium Residential."

(3) Zone Change No. 03-04 to change the site's zoning classification from "Residential Agricultural" (R-A), Area District IV to "Single-Family Residential" (R-1), Area District IA.

LOCATION: 1016 West Greendale Street
REQUEST: The applicant is requesting an addition to an existing 2,119-square foot house. The addition is 1,335 square feet, and the garage addition is a net addition of 146 square feet (for a total of 546 square feet of garage space.) The existing house is two-story. The proposed addition would include first and second story additions. The house with the proposed addition would be 4,000 square feet and two stories. The project is located in the "Single-Family Residential" (R-1) Zone, Area District I.

Study session to initiate and receive input regarding the traffic capacity enhancement project on South Azusa Avenue, from San Bernardino Freeway (I-10) to Temple Avenue, to meet the year 2020 traffic demands.

Request for Planning Commission and public comment regarding the South Azusa Avenue Capacity Enhancement Project and associated environmental document.
